Midterm evaluation (during the semester) is intended solely for the teacher (lecturer/practitioner/supervisor), and is designed to improve communication and student learning. It is recommended to choose 2-3 questions that will be formulated in a constructive manner, publish them to the students, and later address the students' comments, and suggest what changes will be made in response to the comments.

Below are several survey templates with pre-prepared questions. You can choose the type of questionnaire, leave, update, delete and add additional questions depending on the information you want to collect from the students. You can send several different questionnaires or the same questionnaire several times at different times during the semester.

Instructions for installing a questionnaire template

1. Download the appropriate template from the options above to your personal computer.

2. Open the desired course website and activate editing by clicking the "Edit Mode" button in the top right bar.

3. Now click on "Course Management" in the top bar.

4. Select "Restore""Restore" option is in the "Course Settings" column and it is shown in a red rectangle.

5. On the next page, add the file you downloaded from here to the file box under "Upload a Backup File" and then click "Restore" and then "Continue":

6. Now type the course number in the "Search courses" field to indicate to the system where you want to perform the restoration and click Search (magnifying glass). Select and mark the appropriate site from the options that the system will offer you. Click "Continue".

7. Click "Next" on the next two pages and then "Perform restore". The questionnaire will be added to the course site in study unit number 11. You can move the questionnaire to any study unit you choose.

8. Enter the questionnaire and go to the "Settings" tab. Here you can set an opening and closing date for the questionnaire, change the availability to students and more. When finished, save.

9. Now, go to the "Feedback" tab and use the "Edit" button to see the existing questions. You can edit the questions as you wish.

At the end of the process, when you are ready to submit the survey - send a message in the course update forum asking students to come in and fill out the feedback. It is recommended to emphasize the importance of the survey to you and the meaning of their contribution to you.
